Most of this is self explanatory. Affiliation is either a guild, the rune knights, or independent. Class should just be a single letter. General Appearance, clothing, personality, and magic should each be at least a paragraph. History should explain the backstory of the character sufficiently. If that takes one paragraph, great. If that takes a dozen, all good. As long as their backstory is fully explained, it is good enough. If you don't list an item or spell, they don't have it. Plain and simple. Post NPCs in a new thread in the main NPC board, and they will get moved into the proper section once they have been accepted.
